You could use

If you require it to be in done flash - but you'd have to program the application yourself..

Go check out the gang over at Actionscript.org. When I need Flash help I go here...good group.
In their tutorials page they have alot of stuff...including a tutorial on building quizzes...maybe even more than one...

Just do a find on quiz and you'll find some resources.
