I think this is the known bug in pdflib-3.01..please upgrade to pdflib-3.02.

Taken from changes.txt of pdflib-3.02:

- PDF_open_fp() resulted in a crash or PDFlib exception because the file
  pointer got overwritten due to wrong initialization order (p_basic.c).
