Which ldap? openldap?
Please, add a shortest but complete reproducive script here.

Trying to call ldap_mod_* (*=add/del/replace) or ldap_modify
within a script causes apache to dump core.

PHP 4.0.2, compiled as DSO
Apache 1.3.12
Linux 2.2.16-3

PHP compiled with:
--enable-track-vars --disable-posix-threads --with-pgsql
--with-ldap

Using php.ini-dist in /usr/local/lib
