Patches first then commits sounds good as well.

I'm not saying that someone should be sending patches to php-dev for the
rest of their life, but I'm saying that before we hand them (in a large
percent of cases) un-monitored cvs access, lets get to see them send a few
patches and at least somewhat verify that they don't want to hurt PHP (and
that they know C).

The extension specific commit could be a good idea, but it might be
somewhat of a barrier to the process, I think we might add a note in the
CVSRULES file that you should e-mail the author of a extensions before you
make any significantly large commits to that extension. That way it gives
them a chance to look it over (or to just say go ahead I'm too busy).
