I found two bug reports which have EXACTLY my symptoms. Here:
http://www.phpbuilder.com/mail/phplib-list/199911/0369.php?print_mode=1
http://www.geocrawler.com/archives/3/195/1999/3/0/1373601/
This basically makes OOHforms unusable when used with Template.
Is there a fix?
