i would love to post a patch, but first i need to resolve the problem.
Part of this problem is that i can't even reproduce it. i was hoping
some had experienced the same problem.

i'm not sure i understand what you are saying. This is not happening
with the demo and i don't have the query string again=yes.

The only information that might help is that when i hit this problem,
i've poked around with the files. One thing i tried was to turn on
debug in db_mysql.inc. What that did was show 3 debug lines. One
updating the session, one selecting the session and one inserting
another session. i'll have to get that exact information, but i think
it's possible that this is where it is erroring. And to be honest, i am
not sure why it would be inserting another session.
