DSVR - http://www.dsvr.co.uk You can build PHP yourself, full access to
server config etc. I only have one of their free accounts (no PHP...) but
the support is superb and it seems very cheap for what it is. Much better
value than Webfusion :-))

What does LAMP stand for? Linux, apache, ???, PHP I guess :-)
