Get the FORM tag *OUTSIDE* all those TABLE/TR/TD tags. Don't criss-cross
your HTML opening/closing tags ever. Always nest them.

Right:
<FORM><TABLE><TR><TD></TD></TR></TABLE></FORM>
<TABLE><TR><TD></TD><FORM></FORM></TR></TABLE>

Wrong:
<TABLE><TR><TD><FORM></TD></TR></TABLE></FORM>
<TABLE><TR><TD><FORM></TD></TR></FORM></TABLE>
<TABLE><TR><TD><FORM></TD></FORM></TR></TABLE>
